Job Summary

• The Community Health Worker II will support community members by providing outreach, care coordination, and assistance with accessing medical and social services.
• The role involves conducting screenings for Social Determinants of Health (SDOH), connecting individuals with resources such as food, housing, and transportation, and supporting enrollment in health plans.
• The position requires flexibility to work at various clinic locations and community events while building trust and engagement within the community.

Key Responsibilities

• Engage with community members through outreach and resource navigation.
• Build trust and provide education on available health and social service programs.
• Coordinate care by connecting individuals to primary care, specialty care, and social service resources.
• Conduct Social Determinants of Health (SDOH) screenings and assist with health plan enrollment.
• Provide support at multiple work locations, including clinics and community events.

Required Qualifications

• High school diploma or equivalent.
• 1–2 years of experience in community-based health care preferred.
• Proficiency in computer operations and internet-based applications.
• Bilingual ability preferred.

Certifications

• Community Health Worker (CHW) Certification — required.
• Certified Enrollment Counselor (CEC) Certification — required.
