Job Summary
The Clinical Care Coordinator / Mental Health Therapist provides comprehensive behavioral health services in an acute psychiatric setting. This role includes intake assessments, individual, group, and family therapy, patient care coordination, and effective caseload management to support patient recovery and well-being.
Key Responsibilities
- Conduct intake assessments to evaluate patient needs and develop treatment plans
- Provide individual therapy to patients in acute psychiatric care
- Facilitate group therapy sessions to support patient engagement and recovery
- Deliver family therapy to involve families in treatment planning and care
- Coordinate patient care across multidisciplinary teams to ensure continuity of services
- Manage caseloads effectively to meet patient and organizational needs
- Maintain documentation in compliance with regulatory and organizational standards
Required Qualifications
- Master’s degree in Counseling or Social Work
- Active Illinois LCSW license (LPC/LSW may be considered)
- Minimum 1 year of experience (2+ years preferred)
- Strong communication and interpersonal skills
- Must complete a negative drug screen
Preferred Qualifications
- Experience providing individual and group therapy
- Prior work in acute psychiatric or behavioral health settings
Certifications
- LCSW license (required)
- LPC/LSW may be considered
