Registered Nurse - Case Manager

What are the top responsibilities for this position?
- Conduct comprehensive admission and ongoing case management assessments, including medical necessity, level of care validation, and progression of care.
- Drive length-of-stay optimization and access day performance through proactive discharge planning and daily interdisciplinary collaboration.
- Ensure CMS-compliant reviews and form securement, including timely admission reviews, continued stay determinations, IMM and MOON administration.
- Coordinate care transitions, post-acute placement, to prevent avoidable delays and readmissions.
- Maintain accurate, timely clinical documentation supporting medical necessity, care progression, and regulatory requirements.
- Partner with physicians, nursing, social work, and ancillary teams to resolve barriers to discharge and align care plans.
- Support audit readiness and compliance.
- Monitor high-risk populations and identify opportunities for early intervention to reduce excess days.
- Adhere to Northern Light Health policies, Optum standards, and CMS Conditions of Participation.

Bachelor's Degree, ACLS, Maine Nursing license, Must have qualifications/experience: RN license, Case Management, patient facing/bedside presence and Cerner EHR experience; acute hospital setting.
