Job Summary:
The Registered Nurse – Care Manager is responsible for delivering patient-centered care through comprehensive utilization management, case management, and discharge planning. This role ensures that patients across acute care, ambulatory care, extended care, and emergency settings receive timely and appropriate services. The RN Care Manager supports quality outcomes and optimal patient transitions by acting as a liaison between healthcare providers, patients, and families.
Key Responsibilities:
- Patient Assessment and Planning:
- Collaborate with interdisciplinary teams to evaluate patients’ clinical needs and develop effective care plans
- Identify goals and barriers to care while integrating medical, psychosocial, and functional information
Preventative Care:
- Recognize high-risk patients and intervene early to prevent unnecessary readmissions or extended hospital stays
- Educate patients and families about health conditions, medication management, and care follow-up
Discharge Facilitation:
- Develop and implement safe, timely discharge and transition-of-care plans
- Coordinate services with post-acute providers, community resources, and home health agencies
Team Collaboration:
- Serve as a central point of communication between patients, families, physicians, nurses, and support services
- Participate in multidisciplinary rounds and case conferences
Documentation:
- Ensure accurate and timely documentation of assessments, care plans, interventions, and patient outcomes
- Maintain compliance with regulatory, accreditation, and organizational standards
Required Qualifications:
- Education: Associate’s or Bachelor’s degree in Nursing (BSN preferred)
- Licensure: Current Registered Nurse (RN) license in [Insert State]
- Experience: Minimum of 2–3 years of clinical nursing experience, preferably in case management, discharge planning, or acute care
Preferred Qualifications (if any):
