  • Job Summary
    The Social Worker provides comprehensive support to patients and their families within a healthcare setting, addressing emotional, social, and practical needs. This role collaborates with medical teams, ensures continuity of care, and assists patients in understanding treatment plans, accessing resources, and navigating healthcare systems.

    Key Responsibilities
    1. Conduct patient assessments to identify psychosocial, emotional, and environmental needs.
    2. Develop individualized care plans and coordinate services with interdisciplinary healthcare teams.
    3. Provide counseling, crisis intervention, and advocacy for patients and families.
    4. Facilitate access to community resources such as financial assistance, housing, transportation, and support services.
    5. Assist patients in discharge planning, ensuring appropriate placement and follow-up care.
    6. Maintain accurate, timely documentation of assessments, interventions, and care plans.
    7. Participate in care team meetings and contribute to patient-centered decision-making.
    8. Educate patients and families on healthcare processes, treatment options, and available resources.

    Required Qualifications
    1. Bachelor’s or Master’s degree in Social Work (BSW or MSW).
    2. State licensure or eligibility for licensure as a Social Worker.
    3. Experience in a healthcare, hospital, or clinical setting.
    4. Strong communication, counseling, and case management skills.
    5. Ability to work collaboratively with interdisciplinary teams.

    Preferred Qualifications (if any)
    1. Master’s degree in Social Work (MSW) preferred.
    2. Experience in acute care, long-term care, or community health programs.
    3. Bilingual or multilingual skills.

    Certifications (if any)
    1. Licensed Clinical Social Worker (LCSW) or equivalent, if required by state regulations.
    2. Additional certifications in healthcare social work or case management are a plus.
