JOB SUMMARY
We are seeking a dedicated and licensed Physical Therapist to join our outpatient rehabilitation team. This role focuses on providing comprehensive physical therapy services to patients in an outpatient setting, aiding in their recovery and improvement of functional mobility.

Key Responsibilities
• Assess and diagnose physical limitations and movement dysfunction.
• Develop and implement individualized treatment plans to address patient goals.
• Provide therapeutic interventions, including manual therapy, therapeutic exercise, and modalities.
• Educate patients and their families on home exercise programs and injury prevention strategies.
• Document patient progress and treatment according to established guidelines.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.
• Maintain patient records and documentation in an accurate and timely manner.

Schedule / Shift
• Full-time position
• Monday through Friday schedule
• Typical clinic hours

Required Qualifications
• Valid and unrestricted Physical Therapist license in the state of practice.
• Graduation from an accredited physical therapy program.

Experience
• Minimum of 1-2 years of experience in an outpatient physical therapy setting is preferred.

Certifications
• Basic Life Support (BLS) certification required.
