Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice care team. The LPN will provide high-quality nursing care to patients in their homes or care facilities, supporting comfort, dignity, and quality of life during end-of-life care.
Key Responsibilities
- Deliver skilled nursing care under the supervision of a Registered Nurse (RN).
- Monitor patient conditions and report changes promptly to the care team.
- Administer prescribed medications and treatments accurately.
- Provide emotional support to patients and families, fostering a compassionate environment.
- Assist with patient education regarding care plans and symptom management.
- Document patient care activities in compliance with regulatory and organizational standards.
- Collaborate with interdisciplinary team members to ensure comprehensive patient care.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of practice.
- Minimum of 2 year of nursing experience, preferably in hospice, home health, or palliative care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Reliable transportation and willingness to travel to patient homes.
Preferred Qualifications (if any)
- Prior hospice or palliative care experience.
- Familiarity with electronic medical records (EMR) systems.
- Training in pain and symptom management.
Certifications (if any)
- Basic Life Support (BLS) certification required.
- Hospice and Palliative Care Nursing certification (CHPN) preferred.
