Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide high-quality nursing care under the supervision of registered nurses and physicians. The LVN/LPN is responsible for supporting patient care through assessments, medication administration, treatment delivery, patient education, and accurate documentation while ensuring a safe and positive healthcare experience.
Key Responsibilities
- Provide direct patient care in accordance with established nursing standards and care plans.
- Monitor, assess, and document patient conditions, vital signs, and treatment responses.
- Administer medications, injections, and treatments as prescribed by physicians and authorized healthcare providers.
- Assist with patient admissions, transfers, discharges, and ongoing care activities.
- Collect and document patient health information accurately in electronic medical records (EMR).
- Educate patients and family members regarding medications, treatments, and care plans.
- Assist physicians and registered nurses during examinations and procedures.
- Maintain infection prevention and patient safety standards.
- Respond appropriately to changes in patient conditions and report concerns to the appropriate healthcare team members.
- Collaborate with multidisciplinary healthcare teams to ensure quality patient outcomes.
- Ensure compliance with healthcare regulations, facility policies, and nursing standards.
- Maintain a clean, safe, and organized patient care environment.
Required Qualifications
- Active and unrestricted Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the applicable state.
- Graduate of an accredited Practical Nursing or Vocational Nursing program.
- Knowledge of nursing practices, patient care techniques, and clinical procedures.
- Ability to assess patient needs and provide appropriate nursing care.
- Strong communication, interpersonal, and organizational skills.
- Ability to work effectively in a team environment.
- Basic computer proficiency and experience with electronic medical records (EMR).
- Current Basic Life Support (BLS) certification.
Preferred Qualifications
- Previous experience in a hospital, clinic, long-term care, rehabilitation, skilled nursing, home health, or healthcare setting.
- Experience with medication administration, wound care, and patient monitoring.
- Familiarity with EMR systems and healthcare documentation practices.
- Bilingual communication skills are a plus.
- Experience working with geriatric, pediatric, rehabilitation, or acute care patient populations.
Certifications
- Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) License.
- Basic Life Support (BLS) Certification.
- IV Therapy Certification (preferred, where applicable).
- Additional specialty nursing certifications are a plus.
