  • Job Summary  
    We are seeking a dedicated Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide quality care in a long-term rehabilitation setting. This role emphasizes patient comfort and safety while supporting physicians and interdisciplinary teams in delivering effective healthcare services.

    Key Responsibilities
    1. Deliver compassionate and competent nursing care to patients in a rehabilitation environment.
    2. Conduct patient assessments, document care plans, and support chronic condition management.
    3. Assist physicians during office visits, including seating patients, interviewing, and recording vital signs (pulse, blood pressure, height, weight).
    4. Communicate patient needs effectively to physicians, therapists, supervisors, teachers, and family members.
    5. Provide basic patient care and treatments such as taking temperatures, dressing wounds, and monitoring vital signs.
    6. Maintain accurate documentation of patient care and interventions.
    7. Adhere to safety and infection control protocols, including COVID-19 precautions.

    Required Qualifications
    1. Current license to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
    2. Strong communication and interpersonal skills.
    3. Ability to work collaboratively with physicians, therapists, and other healthcare professionals.
    4. Commitment to patient safety, comfort, and quality care.
    5. Must have at least 2 years of recent experience in the specialty within the past 2 years.

    Preferred Qualifications (if any)
    1. Prior experience in long-term care or rehabilitation settings.
    2. Familiarity with electronic health record systems.

    Certifications (if any)
    1. Basic Life Support (BLS) – Required.
    2. Advanced Cardiovascular Life Support (ACLS) – Required.
    3. Valid nursing license (LPN/LVN).
    4. Proof of COVID-19 vaccination (COVID card).
