Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide quality care in a long-term rehabilitation setting. This role emphasizes patient comfort and safety while supporting physicians and interdisciplinary teams in delivering effective healthcare services.
Key Responsibilities
- Deliver compassionate and competent nursing care to patients in a rehabilitation environment.
- Conduct patient assessments, document care plans, and support chronic condition management.
- Assist physicians during office visits, including seating patients, interviewing, and recording vital signs (pulse, blood pressure, height, weight).
- Communicate patient needs effectively to physicians, therapists, supervisors, teachers, and family members.
- Provide basic patient care and treatments such as taking temperatures, dressing wounds, and monitoring vital signs.
- Maintain accurate documentation of patient care and interventions.
- Adhere to safety and infection control protocols, including COVID-19 precautions.
Required Qualifications
- Current license to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Strong communication and interpersonal skills.
- Ability to work collaboratively with physicians, therapists, and other healthcare professionals.
- Commitment to patient safety, comfort, and quality care.
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
Preferred Qualifications (if any)
- Prior experience in long-term care or rehabilitation settings.
- Familiarity with electronic health record systems.
Certifications (if any)
- Basic Life Support (BLS) – Required.
- Advanced Cardiovascular Life Support (ACLS) – Required.
- Valid nursing license (LPN/LVN).
- Proof of COVID-19 vaccination (COVID card).
