Job Summary
We are seeking a dedicated and patient-focused Physical Therapist to evaluate, diagnose, and treat individuals with physical impairments, functional limitations, and movement disorders. The Physical Therapist will develop individualized treatment plans to improve mobility, reduce pain, restore function, and enhance patients' overall quality of life.
Key Responsibilities
- Perform comprehensive patient evaluations and assessments.
- Develop and implement individualized treatment plans based on patient needs and clinical findings.
- Provide therapeutic interventions, exercises, and manual therapy techniques to improve mobility and function.
- Monitor patient progress and modify treatment plans as necessary.
- Educate patients and caregivers on treatment goals, exercises, injury prevention, and home care programs.
- Maintain accurate and timely patient documentation in accordance with organizational and regulatory standards.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Ensure compliance with safety, quality, and professional practice standards.
- Participate in discharge planning and recommend appropriate follow-up care and rehabilitation services.
Required Qualifications
- Graduate of an accredited Physical Therapy program.
- Active and unrestricted Physical Therapist (PT) License in the applicable state.
- Strong clinical assessment, treatment planning, and patient care skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary healthcare team.
- Knowledge of rehabilitation principles and evidence-based treatment practices.
Preferred Qualifications
- Prior experience in a hospital, outpatient, rehabilitation, skilled nursing, or home health setting.
- Experience with electronic medical record (EMR) systems.
- Advanced training or specialization in orthopedic, neurological, sports, or geriatric rehabilitation.
- Doctor of Physical Therapy (DPT) degree preferred.
Certifications
- Physical Therapist (PT) License required.
- Basic Life Support (BLS) Certification preferred or required based on facility policy.
- Specialty certifications (Orthopedic, Neurologic, Sports, Geriatric, or other clinical specialties) preferred.
