  • Job Summary
    We are seeking a compassionate and experienced Registered Nurse Care Manager to join our healthcare team. This role is responsible for coordinating patient care, ensuring quality outcomes, and serving as a liaison between patients, families, and healthcare providers. The Care Manager will play a critical role in guiding patients through their care plans and supporting them in achieving optimal health.

    Key Responsibilities
    1. Assess patient needs and develop individualized care plans
    2. Coordinate care across multiple providers and healthcare settings
    3. Monitor patient progress and adjust care plans as needed
    4. Educate patients and families on treatment options, medications, and lifestyle changes
    5. Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care
    6. Document patient interactions and maintain accurate medical records

    Required Qualifications
    1. Active Registered Nurse (RN) license in the state of practice
    2. Bachelor of Science in Nursing (BSN) or equivalent degree
    3. Must have at least 2 years of recent experience in the specialty within the past 2 years.
    4. Strong communication, organizational, and problem-solving skills
    5. Ability to work independently and as part of a multidisciplinary team

    Preferred Qualifications (if any)
    1. Experience in case management, care coordination, or utilization review
    2. Knowledge of chronic disease management and population health strategies
    3. Prior work in managed care or insurance-based healthcare settings

    Certifications (if any)
    1. Case Management Certification (CCM) preferred
    2. Basic Life Support (BLS) certification required
