  • Job Summary  
    The Clinical LPN/LVN provides direct patient care, supports chronic disease management, and collaborates with providers and staff to ensure efficient clinic operations. This role involves performing health assessments, administering medications, documenting patient records, and participating in outreach initiatives to promote preventive health behaviors.

    Key Responsibilities
    1. Direct Patient Care: Perform health assessments, screenings, immunizations, wound care, and medication administration as prescribed by providers.
    2. Chronic Disease Management: Educate patients on managing chronic conditions and promote preventive health practices.
    3. Collaboration & Support: Work alongside providers, behavioral health specialists, and staff to coordinate patient care and assist with clinic workflow.
    4. Documentation: Maintain accurate electronic medical records (EMR) in compliance with safety and regulatory standards.
    5. Outreach & Counseling: Conduct telephone triaging, patient counseling, and participate in community health initiatives.

    Required Qualifications & Skills
    1. Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the State of California.
    2. Previous experience in a community health center, primary care, or outpatient setting preferred.
    3. Strong communication skills and ability to work independently in a fast-paced environment.
    4. Competency in handling diverse patient populations.
    5. Must have at least 2 years of recent experience in the specialty within the past 2 years.
    Certifications
    1. CPR/BLS Certification required.
