Job Description:

  • Job Summary 
    The Social Worker is responsible for conducting psychosocial assessments, developing individualized care plans, and coordinating safe and timely discharge planning. This role provides crisis counseling, emotional support, and connects patients and families with essential community resources to promote overall well-being and recovery.

    Key Responsibilities
    1. Conduct psychosocial assessments to identify patient needs, risks, coping mechanisms, and support systems.
    2. Develop individualized care plans in collaboration with the interdisciplinary healthcare team.
    3. Coordinate safe and timely discharge plans, including home care, rehabilitation, skilled nursing facilities (SNF), long-term acute care (LTAC), and hospice services.
    4. Provide crisis counseling for emotional distress, trauma, grief, and acute mental health issues.
    5. Connect patients and families with community resources such as financial aid, housing, food support, behavioral health services, and social support programs.

    Required Qualifications
    1. Bachelor’s or Master’s degree in Social Work or related field.
    2. Strong knowledge of psychosocial assessment techniques and care planning.
    3. Excellent communication, counseling, and interpersonal skills.
    4. Ability to work collaboratively within an interdisciplinary team.
    5. Must have at least 2 years of recent experience in the specialty within the past 2 years.

    Preferred Qualifications (if any)
    1. Prior experience in healthcare or hospital-based social work.
    2. Familiarity with discharge planning and community resource coordination.

    Certifications (if any)
    1. Basic Life Support (BLS) certification (Required).
    2. Crisis Prevention Intervention (CPI) certification (Required).
