JOB SUMMARY
Provide comprehensive physical therapy services to a diverse patient population within an acute care hospital setting, including specialized care in an Inpatient Rehabilitation Facility (IRF). Focus on evaluation, treatment planning, intervention, and patient/family education to optimize functional mobility and recovery.

Key Responsibilities
• Conduct thorough physical therapy evaluations for patients across various units, including the IRF.
• Develop and implement individualized treatment plans based on patient needs, goals, and diagnoses.
• Administer therapeutic interventions utilizing specialized equipment and techniques, such as Vector and LiteGait.
• Provide patient and family education regarding home exercise programs, adaptive strategies, and equipment use.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated and effective patient care.
• Participate in team conferences and discharge planning to facilitate smooth transitions of care.
• Document all patient assessments, interventions, and progress notes in the electronic medical record.
• Adhere to all hospital policies, procedures, and quality standards.

Schedule / Shift
• Monday - Friday
• 8:00 AM - 4:30 PM

Required Qualifications
• Active Physical Therapist (PT) license.
• Active BLS certification.
• Willingness to work in the Inpatient Rehabilitation Facility (IRF) unit.
• Strong critical thinking and clinical reasoning skills.

Preferred Qualifications
• Previous hospital and/or IRF experience.
• Experience with stroke and/or neuro-rehabilitation.

• BLS

Experience
• Patient Ratios: 4 patients/day (IRF), 8-12 patients/day (acute care).
• Experience with adaptive devices and equipment.
• Willingness to work 1 weekend shift per month.
