  • Job Summary
    The Registered Nurse (RN) in the Transitional Care Unit (TCU) is responsible for delivering high-quality, patient-centered care to individuals transitioning from acute hospital care to home or long-term care settings. The RN works collaboratively with an interdisciplinary team to manage complex patient needs, promote recovery, and prevent hospital readmissions. This role is vital in providing comprehensive assessments, care planning, and education to ensure smooth transitions and optimal outcomes.

    Key Responsibilities
    1. Perform comprehensive nursing assessments, develop individualized care plans, and implement nursing interventions.
    2. Monitor patient progress and adjust care plans as necessary to meet changing health status.
    3. Administer medications and treatments in accordance with physician orders and nursing standards.
    4. Coordinate discharge planning and collaborate with physicians, case managers, therapists, and families to ensure appropriate post-discharge care.
    5. Provide patient and family education regarding diagnoses, medications, and recovery plans.
    6. Maintain accurate, timely, and complete documentation in the electronic medical record.
    7. Respond to changes in patient condition promptly and communicate with the healthcare team effectively.
    8. Adhere to infection control practices, hospital policies, and regulatory standards.
    9. Participate in quality improvement initiatives and contribute to a culture of patient safety and excellence.

    Required Qualifications
    1. Graduate of an accredited school of nursing.
    2. Active and unrestricted Registered Nurse (RN) license.
    3. Minimum of 1–2 years of clinical nursing experience, preferably in a medical-surgical, rehab, or transitional care setting.
    4. Strong clinical judgment and assessment skills.
    5. Ability to work both independently and collaboratively in a multidisciplinary environment.

    Preferred Qualifications
    1. Prior experience in transitional care, rehabilitation, or long-term care.
    2. Experience with discharge planning and care coordination.

    Certifications
    1. Basic Life Support (BLS) – Required
    2. Advanced Cardiovascular Life Support (ACLS) – Preferred
