  • Position Overview
    We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our healthcare team in Arlington, VA. The ideal candidate will provide psychosocial assessments, counseling, crisis intervention, discharge planning, and community resource coordination for patients and their families. This role requires strong communication skills, collaboration with interdisciplinary teams, and a commitment to patient-centered care.

    Key Responsibilities
    1. Conduct psychosocial assessments of patients and families.
    2. Develop individualized care plans and discharge strategies.
    3. Provide crisis intervention and emotional support services.
    4. Coordinate community referrals and support programs.
    5. Collaborate with healthcare teams including physicians, nurses, and case managers.
    6. Advocate for patient needs across social, emotional, and behavioral health.
    7. Document interventions and outcomes accurately in the electronic medical record.
    8. Assist patients and families in navigating healthcare systems and available services.
    9. Ensure compliance with facility policies, professional standards, and regulatory requirements.

    Required Qualifications
    • Master’s Degree in Social Work (MSW) from an accredited institution.
    • Active Virginia Licensed Clinical Social Worker (LCSW) license.
    • Previous clinical social work experience in a hospital or healthcare setting preferred.
    • Strong knowledge of case management, discharge planning, and community resources.
    • Excellent communication, organizational, and interpersonal skills.
    • Ability to work effectively in a fast-paced healthcare environment.

    Preferred Qualifications
    • Experience with interdisciplinary care coordination.
    • Familiarity with local community resources and support programs.

    Certifications
    • State LCSW license (required).
