Job Summary
The Skilled Nursing Facility (SNF) Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents in accordance with physician orders, facility policies, and state regulations. This role ensures safe, effective, and compassionate care while supporting the interdisciplinary healthcare team.
Key Responsibilities
- Administer medications and treatments in accordance with physician orders.
- Carry out and document physician orders accurately and in a timely manner.
- Monitor and record residents’ vital signs.
- Provide G-tube feedings and related care as prescribed.
- Notify supervisors promptly of any changes in resident condition or concerns.
- Assist in guiding Certified Nursing Assistants (CNAs) with assignments and provide support as needed.
Required Qualifications
- Minimum of 1 year of recent nursing experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting.
- Must have at least 2 years of recent experience in the specialty within the past 2 years.Current Basic Life Support (BLS) certification.
- Active LPN license in good standing.
Preferred Qualifications
- Experience with electronic health record (EHR) systems.
- Strong communication and teamwork skills.
- Prior experience mentoring or supporting CNAs.
Certifications (if any)
- Licensed Practical Nurse (LPN) license (required).
- Basic Life Support (BLS) certification (required).
