JOB SUMMARY Licensed Practical Nurse (LPN) responsible for providing direct nursing care to residents in a Long Term Care (LTC) setting.
Key Responsibilities - Administer medications and treatments as prescribed by physicians. - Monitor resident vital signs and health status, reporting any changes to the Director of Nursing or Charge Nurse. - Provide personal care, including bathing, dressing, and feeding, as needed. - Document resident care, observations, and interventions accurately and timely in the electronic health record. - Assist with resident activities and therapeutic programs. - Maintain a safe and clean environment for residents. - Respond to resident emergencies and provide appropriate care. - Collaborate with the interdisciplinary healthcare team to develop and implement resident care plans.
Schedule / Shift - Night Shift
Required Qualifications - Valid Licensed Practical Nurse (LPN) license in Colorado. - High School Diploma or GED.
Certifications - Basic Life Support (BLS) certification.
Experience - Minimum of 1 year of experience working as an LPN.
Skills - Proficiency in medication administration. - Strong assessment and critical thinking skills. - Excellent communication and interpersonal skills. - Ability to work effectively as part of a team. - Compassionate and patient-centered approach to care.
EMR / Systems - Experience with electronic health record (EHR) systems.
Patient Population - Geriatric residents in a Long Term Care setting.
