  • Job Summary
    The  RN Case Manager is responsible for delivering skilled nursing care and comprehensive case management to patients and families in accordance with the State Nurse Practice Act, professional standards, and agency protocols. The RN Case Manager promotes patient health through direct clinical care, education, counseling, and rehabilitative support. This role requires clinical judgment, compassionate communication, and the ability to work independently in a home-based setting. The position reports directly to the VNA Clinical Team Manager.

    Key Responsibilities
    • Provide professional nursing care to patients in their homes using nursing theory, process, and clinical judgment.
    • Perform patient assessments, develop and implement care plans, and evaluate patient outcomes.
    • Educate patients and families on treatment plans, medications, and health maintenance strategies.
    • Document all patient care activities, assessments, and changes in condition accurately and in a timely manner.
    • Coordinate care with interdisciplinary team members including physicians, therapists, and social workers.
    • Promote continuity of care by communicating effectively with healthcare providers, patients, and family members.
    • Advocate for patients and help identify community resources as needed.
    • Participate in team meetings, in-service training, and quality improvement initiatives.
    • Ensure all practices comply with regulatory and safety standards.
    Required Qualifications
    • Graduate of an accredited school of professional nursing.
    • Current Registered Nurse (RN) license in the Commonwealth of Massachusetts.
    • Current BLS (Basic Life Support) certification through the American Heart Association.
    • Minimum of 1 year of Medical/Surgical nursing experience.
    • Valid driver’s license, good driving record, and reliable transportation.
    Preferred Qualifications (if any)
    • Bachelor of Science in Nursing (BSN).
    • Previous experience in home care or hospice nursing.
    • Experience with electronic medical records (EMR) and mobile documentation tools.

    Certifications (if any)
    • Registered Nurse (RN) – Active state license.
    • BLS Certification – American Heart Association.
