  • Job Summary
    We are seeking a compassionate and experienced Home Hospice Registered Nurse to provide end-of-life care and support to patients and their families in a home-based setting. The ideal candidate will demonstrate exceptional clinical skills, emotional intelligence, and independence in managing hospice caseloads. This role focuses on symptom management, patient education, and ensuring quality, dignified care during the final stages of life.

    Key Responsibilities
    1. Conduct comprehensive patient assessments and develop individualized care plans.
    2. Provide pain and symptom management in alignment with hospice protocols.
    3. Deliver compassionate education and emotional support to patients and families.
    4. Document patient progress and care details accurately within the EMR system.
    5. Collaborate with the interdisciplinary hospice team, including physicians, social workers, and chaplains.
    6. Ensure that documentation for hospice certifications meets regulatory and organizational standards.
    7. Maintain professional communication and coordination with caregivers and healthcare partners.
    8. Demonstrate strong time management and the ability to work independently in the field.

    Required Qualifications
    1. Active Registered Nurse (RN) license in the state of practice.
    2. Hospice experience required.
    3. Proficiency with Electronic Medical Records (EMR).
    4. Valid Driver’s License and reliable transportation.
    5. Strong understanding of documentation requirements for hospice certifications.

    Preferred Qualifications (if any)
    1. Prior home health experience.
    2. Previous hospice case management experience.
    3. Demonstrated ability to work independently and manage complex caseloads.

    Certifications (if any)
    1. RN license – Required.
    2. CPR certification – Required.
