Job Summary
The Licensed Practical Nurse (LPN) – Rehab provides basic nursing care under the supervision of a Registered Nurse (RN) or physician in alignment with individualized rehabilitation care plans. The LPN plays a key role in supporting patients through their recovery journey by assisting with daily activities, administering medications, and monitoring patient progress in a rehabilitative setting.
Key Responsibilities
- Deliver basic nursing care as directed by an RN or physician
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting
- Monitor and record vital signs, weight, and report on patient progress
- Administer oral and injectable medications as prescribed
- Perform wound care, dressing changes, and skin assessments
- Assist with patient mobility, transfers, and range-of-motion exercises
- Observe and promptly report any changes in patient condition to the supervising nurse or physician
Required Qualifications
- Current and valid LPN license
- CPR/BLS certification
- Strong understanding of nursing best practices within a rehabilitation setting
- Excellent interpersonal and communication skills
- Ability to work effectively as part of a multidisciplinary team
- Attention to detail and strong documentation skills
Preferred Qualifications
- Previous experience in a rehabilitation or long-term care facility
- Familiarity with electronic health record (EHR) systems
Certifications
- Licensed Practical Nurse (LPN) – Required
- BLS (Basic Life Support) – Required
