Job Summary
The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Rehabilitation provides direct patient care in a rehabilitation setting. This role supports patients who are short-stay and more acute than those typically found in skilled nursing facilities (SNFs). The LVN/LPN works collaboratively with the healthcare team to ensure safe, effective, and compassionate care that promotes recovery and independence.
Key Responsibilities
- Provide direct nursing care including patient assessments, medication administration, and treatment interventions.
- Monitor patient conditions and promptly report changes to the supervising RN or physician.
- Assist patients with rehabilitation activities and daily living needs.
- Educate patients and families on care plans, recovery goals, and discharge instructions.
- Maintain accurate documentation in compliance with facility and regulatory standards.
- Support infection control, patient safety, and quality improvement initiatives.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
Required Qualifications
- Active LVN/LPN license in a compact state license.
- Basic Life Support (BLS) certification.
- Minimum of 1 year of nursing experience, preferably in rehabilitation or acute care.
- Strong clinical assessment and communication skills.
- Ability to work 3x12-hour shifts (36 hours per week) with every other weekend required.
Preferred Qualifications (if any)
- Previous experience in rehabilitation or short-stay patient care.
- Familiarity with electronic health records (EHR) systems.
Certifications (if any)
- BLS certification required.
- Additional specialty certifications in rehabilitation nursing are a plus.
