  • Job Summary
    The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Rehabilitation provides direct patient care in a rehabilitation setting. This role supports patients who are short-stay and more acute than those typically found in skilled nursing facilities (SNFs). The LVN/LPN works collaboratively with the healthcare team to ensure safe, effective, and compassionate care that promotes recovery and independence.

    Key Responsibilities
    1. Provide direct nursing care including patient assessments, medication administration, and treatment interventions.
    2. Monitor patient conditions and promptly report changes to the supervising RN or physician.
    3. Assist patients with rehabilitation activities and daily living needs.
    4. Educate patients and families on care plans, recovery goals, and discharge instructions.
    5. Maintain accurate documentation in compliance with facility and regulatory standards.
    6. Support infection control, patient safety, and quality improvement initiatives.
    7. Collaborate with interdisciplinary teams to optimize patient outcomes.

    Required Qualifications
    1. Active LVN/LPN license in a compact state license.
    2. Basic Life Support (BLS) certification.
    3. Minimum of 1 year of nursing experience, preferably in rehabilitation or acute care.
    4. Strong clinical assessment and communication skills.
    5. Ability to work 3x12-hour shifts (36 hours per week) with every other weekend required.

    Preferred Qualifications (if any)
    1. Previous experience in rehabilitation or short-stay patient care.
    2. Familiarity with electronic health records (EHR) systems.

    Certifications (if any)
    1. BLS certification required.
    2. Additional specialty certifications in rehabilitation nursing are a plus.
