  • Job Summary
    The Clinical Social Worker/Case Manager will support the Behavioral Health Home Hospital (BHH) program, an innovative care delivery model that provides highly specialized psychiatric services to patients in their homes as an alternative to inpatient psychiatric hospitalization. This role delivers quality clinical services in community settings, ensuring care aligns with organizational philosophy, policy, and standards of nursing and community health practice. The position is part of a research trial beginning January 5, offering temporary, part-time work with an anticipated schedule of approximately one hour per day depending on caseload.

    Key Responsibilities
    1. Provide clinical social work services to patients in their homes, temporary residences, or remotely.
    2. Conduct psychosocial assessments and develop individualized treatment interventions.
    3. Collaborate with interdisciplinary teams to create and implement master treatment plans.
    4. Deliver hospital-level psychiatric care in community settings for up to 4 patients at a time.
    5. Address psychosocial, environmental, age-specific, and cultural issues to maximize patient well-being.
    6. Utilize social work techniques and theory to provide consultation, treatment, and advocacy.
    7. Maintain thorough and timely electronic documentation in compliance with hospital policies.
    8. Communicate effectively with patients, families, and healthcare professionals to ensure coordinated care.
    9. Participate in research trial activities to broaden treatment opportunities for patients with acute psychiatric conditions.

    Required Qualifications
    1. Master’s degree in Social Work or related field.
    2. Active state licensure as a Clinical Social Worker.
    3. Prior experience in psychiatric or behavioral health settings.
    4. Strong knowledge of clinical social work techniques, assessment, and intervention strategies.
    5. Excellent communication, documentation, and interdisciplinary collaboration skills.

    Preferred Qualifications (if any)
    1. Experience providing care in community or home-based settings.
    2. Familiarity with electronic health record (EHR) systems.
    3. Prior involvement in research trials or innovative care delivery models.

    Certifications (if any)
    1. CPR or BLS certification may be required depending on facility standards.
