Job Summary
The Medical Social Worker – Home Health provides emotional support, resource counseling, and care coordination to patients and families receiving home health services. This role focuses on helping patients cope with illness, access community resources, and develop plans to meet their social, emotional, and healthcare needs while promoting independence and quality of life in the home setting.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for home health patients to identify emotional, social, and financial needs.
- Develop individualized care plans in collaboration with the interdisciplinary healthcare team, patients, and families.
- Provide supportive counseling to patients and families dealing with illness, adjustment issues, or end-of-life concerns.
- Coordinate access to community resources, including financial assistance, support groups, transportation, and home services.
- Assist patients and families in understanding and implementing medical care plans.
- Advocate for patient rights and ensure care is provided in accordance with ethical and regulatory standards.
- Maintain accurate and timely documentation in the electronic medical record (EMR) and complete all required reports.
- Participate in team meetings and case conferences to support care planning and quality outcomes.
Required Qualifications
- Master’s degree in Social Work (MSW) from an accredited program.
- Active state licensure as a Medical or Clinical Social Worker (as required by state regulations).
- Minimum of 1 year of experience in a healthcare or home health setting.
- Strong communication, problem-solving, and organizational skills.
Preferred Qualifications (if any)
- Experience in home health, hospice, or community-based care.
- Knowledge of Medicare, Medicaid, and community resource programs.
- Familiarity with electronic health record (EHR/EMR) systems.
Certifications (if any)
- Basic Life Support (BLS) certification – preferred or required by some agencies.
- Licensed Clinical Social Worker (LCSW) – preferred.
