    JOB SUMMARY
    Provide comprehensive physical therapy services to patients in a rehabilitation and skilled nursing setting. Assess, plan, implement, and evaluate patient care to restore function, maximize mobility, and promote overall well-being.

    Key Responsibilities
    - Evaluate patients' physical condition, functional abilities, and limitations.
    - Develop individualized treatment plans based on patient assessments and goals.
    - Administer therapeutic interventions, including exercises, manual therapy, and therapeutic modalities.
    - Educate patients and their families on home exercise programs and adaptive strategies.
    - Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care.
    - Document patient progress, treatment efficacy, and outcomes accurately and timely.
    - Maintain patient records in compliance with facility policies and regulatory standards.
    - Participate in patient care conferences and staff meetings.
    - Maintain professional competency through continuing education.

    Required Qualifications
    - Graduate of an accredited Physical Therapy program.
    - State licensure as a Physical Therapist.

    Certifications
    - Basic Life Support (BLS) certification.

    Experience
    - Experience in a skilled nursing or sub-acute rehabilitation setting is preferred.

    Skills
    - Strong clinical assessment and treatment planning skills.
    - Proficiency in therapeutic exercise and manual therapy techniques.
    - Excellent communication and interpersonal skills.
    - Ability to work independently and as part of a team.
    - Strong documentation and organizational skills.

    EMR / Systems
    - Electronic Health Record (EHR) charting experience.

    Patient Population
    - Geriatric patients
    - Post-operative patients
    - Patients with orthopedic, neurological, and cardiopulmonary conditions

    Unit / Specialty
    - Rehabilitation and Skilled Nursing Facility
