Job Summary
The Physical Therapist is a vital member of the interdisciplinary team, responsible for delivering high-quality physical therapy services through comprehensive evaluation, program planning, and treatment administration. This role involves performing detailed clinical assessments, developing age-appropriate care plans, and ensuring accurate documentation of patient progress. Flexibility and responsiveness to new and changing work assignments are essential.
Key Responsibilities
- Provide quality physical therapy services to patients based on physician referrals.
- Conduct detailed clinical assessments to evaluate individual patient needs.
- Develop and implement comprehensive, age-appropriate plans of care following evaluations.
- Collaborate with the interdisciplinary team to ensure coordinated patient care.
- Accurately document patient treatments and progress in compliance with standards.
- Adapt effectively to new and changing work assignments.
Required Qualifications
- Must reside within 100 miles of the facility.
- Minimum of 2 year of inpatient rehab or acute care experience.
- Degree in Physical Therapy.
- Current PT License.
- State licensure required.
- Basic Life Support (BLS) certification.
- Crisis Prevention Training (TEAM) required within 6 months of hire (PRN not applicable).
- Primary Source Online Verification required.
Preferred Qualifications (if any)
- First Aid certification.
- Prior experience in diverse rehabilitation settings.
