Job Summary
The Certified Home Health/Hospice Aide (CHHA) works under the supervision of a Registered Nurse or therapist to provide personal care services that support patient comfort, dignity, and independence. This role focuses on assisting patients in their homes, documenting observations, promptly reporting changes, and ensuring continuity and safety in care. Duties do not include nursing practice activities as defined by the state Nurse Practice Act (e.g., administering medication, taking orders, or performing sterile procedures).
Key Responsibilities
- Provide personal care services to patients in home health and hospice settings.
- Document patient observations and promptly report changes to supervising clinicians.
- Assist patients with daily living activities to promote comfort and independence.
- Support infection control protocols and maintain a safe environment.
- Demonstrate empathy and professionalism when working with diverse patient populations, including the terminally ill.
- Maintain confidentiality and adhere to healthcare compliance standards.
- Participate in hospice-specific training and supervised practical experience.
Required Qualifications
- High School Diploma or equivalent.
- Experience providing personal care in home health and/or hospice settings.
- Ability to work independently in the field while maintaining professionalism in patient homes.
- Strong interpersonal, communication, and documentation skills.
- Competency with equipment and technology used for patient care.
- Knowledge of infection control protocols.
- Demonstrated empathy, calm demeanor, and comfort supporting terminally ill patients.
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
Preferred Qualifications
- Prior hospice-specific training and experience.
- Experience working with diverse patient populations.
Certifications & Licensure
- Active Certified Home Health Aide (CHHA) certification in California (in good standing).
- Current Basic Life Support (BLS) certification.
- Registered on the California Home Care Aide Registry (CA DSS – Home Care Services Bureau).
- Valid driver’s license with insured, reliable vehicle.

Physical Requirements
