Job Summary
The Certified Nursing Assistant (CNA) – Hospice provides compassionate personal care and supportive services to hospice patients under the direction of a registered nurse or supervising clinical team. This role focuses on maintaining patient comfort, dignity, and quality of life while assisting with daily living activities in accordance with individualized care plans.
The ideal candidate will demonstrate empathy, strong communication skills, and the ability to provide high-quality patient care within hospice or end-of-life care environments.
Key Responsibilities
- Provide direct patient care and assistance with activities of daily living, including bathing, grooming, dressing, toileting, and mobility support.
- Assist patients with transfers, positioning, ambulation, and range-of-motion exercises as directed by the care plan.
- Monitor and report changes in patient condition, behavior, or physical status to the supervising nurse or clinical team.
- Support patient comfort and emotional well-being while maintaining dignity and respect.
- Assist with meal preparation, feeding, hydration, and light housekeeping tasks as appropriate.
- Maintain accurate documentation of care provided and patient observations in accordance with organizational policies.
- Follow infection control, safety, and confidentiality standards at all times.
- Collaborate with nurses, caregivers, family members, and interdisciplinary care teams to support patient-centered care.
- Provide compassionate support to patients and families during end-of-life care.
- Maintain a clean, safe, and organized patient environment.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification in the applicable state.
- Current CPR/BLS certification, if required by state or employer guidelines.
- Experience providing direct patient care in hospice, home health, long-term care, hospital, or related healthcare settings.
- Knowledge of basic patient care techniques, infection control, and safety procedures.
- Ability to communicate effectively with patients, families, and healthcare teams.
- Strong compassion, empathy, and interpersonal skills.
- Ability to maintain patient confidentiality and professionalism.
- Physical ability to assist with lifting, transferring, and supporting patient mobility needs.
- Strong organizational and time management skills.
Preferred Qualifications
- Prior hospice or palliative care experience.
- Experience working in home health or end-of-life care environments.
- Familiarity with electronic medical records (EMR) or healthcare documentation systems.
