Job Summary
The Acute Rehab RN provides comprehensive nursing care to patients recovering from complex medical, surgical, and neurological conditions requiring physical rehabilitation. The RN facilitates a collaborative, interdisciplinary approach to maximize patient independence, functional improvement, and safe discharge planning. This is a fast-paced unit focused on restoring function and improving the quality of life for patients transitioning from acute hospitalization to home or long-term care.
Key Responsibilities
- Deliver direct patient care based on individualized rehabilitation plans.
- Perform comprehensive assessments and ongoing evaluations of patients’ physical, emotional, and educational needs.
- Collaborate with an interdisciplinary team including physical, occupational, and speech therapists, case managers, and physicians.
- Administer medications, monitor vital signs, and manage wound care, IV therapies, and other medical interventions.
- Promote patient engagement in self-care and functional mobility activities.
- Educate patients and families on care plans, discharge instructions, and recovery goals.
- Document all patient care accurately and timely in accordance with hospital policies and regulatory requirements.
- Participate in care conferences and patient-centered goal setting.
- Respond to changes in patient condition and communicate concerns effectively to providers.
Required Qualifications
- Current RN license in the state.
- Basic Life Support (BLS) certification
- 1–2 years of recent acute care or rehabilitation nursing experience
- Strong clinical skills in medical-surgical or post-acute settings
- Proficient in managing multiple patients in a dynamic environment
- Excellent communication and patient education skills
Preferred Qualifications
- Advanced Cardiac Life Support (ACLS) certification
- Experience in acute inpatient rehab or physical medicine and rehabilitation
- Knowledge of stroke, spinal cord injury, amputation, or orthopedic recovery protocols
- Certified Rehabilitation Registered Nurse (CRRN) credential
