    JOB SUMMARY
    Provide occupational therapy services to patients in their place of residence who have impairments, cognitive and/or physical resulting from injury, disease, disability or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Manager technical, environmental and financial resources.

    Key Responsibilities
    - Assess, plan, implement, and evaluate patient care plans.
    - Provide occupational therapy interventions to improve patients' functional abilities.
    - Educate patients and families on home exercise programs and adaptive techniques.
    - Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
    - Document patient progress and outcomes accurately and in a timely manner.
    - Manage technical, environmental, and financial resources related to patient care.

    Schedule / Shift
    - Travel required within PHH's geographic area.

    Required Qualifications
    - Licensed Occupational Therapist in Massachusetts.
    - Ability to work with various computer software.
    - Strong customer service skills.
    - Ability to work with diverse populations.
    - Excellent verbal, written, and electronic communication skills.
    - Ability to prioritize work and adapt to changing situations.

    Experience
    - Two years of experience in acute care, rehab, or homecare as an Occupational Therapist.

    Certifications
    - Certified Home Care Occupational Therapist.

    Skills
    - Patient assessment and treatment planning.
    - Therapeutic intervention techniques.
    - Patient and family education.
    - Interdisciplinary collaboration.
    - Electronic health record (EHR) documentation.
    - Customer service.
    - Communication (verbal, written, electronic).
    - Prioritization and adaptability.

    EMR / Systems
    - Knowledge of hand-held patient care computer devices preferred.
