Job Summary
The Registered Nurse (RN) – Skilled Nursing Facility provides high-quality nursing care to residents in a post-acute or long-term care setting. This role is responsible for assessing residents’ health status, administering medications and treatments, and coordinating care to promote optimal recovery and well-being. The RN works closely with physicians, licensed practical nurses, certified nursing assistants, and families to ensure a safe, compassionate, and supportive environment.
Key Responsibilities
- Assess, plan, implement, and evaluate individualized care plans for each resident.
- Administer prescribed medications and treatments accurately and in accordance with facility policies.
- Monitor residents for changes in condition and report findings to physicians and other healthcare professionals.
- Supervise and guide licensed and unlicensed nursing staff to ensure high-quality care.
- Maintain accurate and timely documentation in residents’ medical records.
- Assist with admission, transfer, and discharge processes.
- Ensure compliance with infection control standards, safety procedures, and regulatory requirements.
- Communicate effectively with residents, families, and interdisciplinary team members to support overall care goals.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
- Minimum of 1 year of clinical nursing experience, preferably in a long-term care or skilled nursing facility.
- Strong assessment, critical thinking, and leadership skills.
Preferred Qualifications (if any)
- Prior experience in rehabilitation or geriatric nursing.
- Knowledge of state and federal regulations related to long-term care.
- Experience with electronic health record (EHR/EMR) systems.
Certifications (if any)
- Basic Life Support (BLS) certification – required.
- Advanced Cardiovascular Life Support (ACLS) certification – preferred.
- Gerontological Nursing Certification (RN-BC) – preferred.
