    JOB SUMMARY
    Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.

    Key Responsibilities
    • Evaluate and treat patients in their homes to address physical impairments, functional limitations, and disabilities.
    • Develop and implement individualized treatment plans in collaboration with patients, families, and healthcare providers.
    • Educate patients and families on home exercise programs and adaptive techniques.
    • Maintain comprehensive and timely electronic documentation of patient care.
    • Communicate effectively with physicians and other allied health professionals to coordinate patient care.
    • Travel to assigned patient cases.

    Schedule / Shift
    Weekend rotations are once every 6 weeks.

    Required Qualifications
    • Physical Therapist Professional license in Massachusetts.
    • 1 year Home Care experience required. Experience in hospice or private care alone does not meet the qualifications.
    • 1 year Case Management Experience within a certified home health agency.

    Experience
    • 1 Year Home Care experience required.
    • 1 Year Case Management Experience required.

    Skills
    • Ability to travel to cases as assigned.
    • Thorough and timely electronic documentation.
    • Interdisciplinary communication.

    EMR / Systems
    • Utilizing thorough and timely electronic documentation.

    Patient Population
    Patients may range in age from birth to over 100 years.

    Unit / Specialty
    Home Health

    Floating / On-Call
    Weekend rotations are once every 6 weeks.
