Job Summary
We are seeking a motivated and experienced Registered Nurse for a Case Management assignment within an acute care setting. The RN Case Manager will be responsible for coordinating patient care from admission to discharge, ensuring effective resource utilization, and supporting optimal patient outcomes. This position is ideal for a self-directed nurse with strong clinical and communication skills who thrives in a collaborative, fast-paced environment.
Key Responsibilities
- Perform comprehensive assessments of patients’ medical, psychosocial, and discharge planning needs.
- Develop and implement individualized case management plans in collaboration with interdisciplinary teams.
- Coordinate transitions of care, including discharge planning, referrals, and post-acute services.
- Monitor and evaluate patient progress to ensure appropriate care delivery and effective use of resources.
- Communicate regularly with physicians, patients, families, and care providers to facilitate continuity of care.
- Document all case management activities in the electronic health record system.
- Act as an advocate for patients and their families to support informed decision-making and care coordination.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice or conditional license (see note below).
- Minimum of 1 year of recent experience in acute care case management.
- Strong knowledge of hospital-based discharge planning and care coordination.
- Excellent problem-solving, time management, and communication skills.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Previous travel nursing or contract experience in case management.
- Familiarity with utilization review, insurance verification, and payer requirements.
Certifications
- Basic Life Support (BLS) – required.
- Case Management Certification (CCM or ACM) – preferred.
