Job Summary
The Registered Nurse (Hospice) provides compassionate, patient-centered care to individuals and families facing end-of-life needs. This role ensures quality clinical care, emotional support, and coordination of services in alignment with hospice philosophy and standards.
Key Responsibilities
- Deliver skilled nursing care to hospice patients in homes, facilities, or inpatient settings.
- Assess patient conditions and develop individualized care plans in collaboration with the interdisciplinary team.
- Administer medications and treatments as prescribed.
- Provide education and emotional support to patients and families regarding disease process, symptom management, and end-of-life care.
- Coordinate care with physicians, social workers, chaplains, and other team members.
- Document patient assessments, interventions, and outcomes in compliance with regulatory and organizational standards.
- Participate in on-call rotation to ensure continuous patient support.
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 1–2 years of clinical nursing experience.
- Strong communication and interpersonal skills.
- Ability to work independently and manage multiple patients.
- Commitment to providing compassionate, dignified care.
Preferred Qualifications (if any)
- Prior experience in hospice or palliative care.
- Familiarity with electronic health record (EHR) systems.
- Training in pain management and symptom control.
Certifications (if any)
- Basic Life Support (BLS) certification required.
- Hospice and Palliative Care Nursing Certification (CHPN) preferred.
