Description
We are seeking a dedicated and compassionate Social Worker to join our healthcare and community services team. The successful candidate will provide comprehensive social work support to individuals and families, facilitating access to resources, offering counseling, and supporting mental health and behavioral needs. This role is integral to enhancing patient care, promoting well-being, and ensuring effective case management across diverse populations. The ideal applicant will possess a strong foundation in clinical counseling, behavioral health, and community outreach, with a commitment to delivering empathetic, evidence-based support.
Duties
- Lead crisis intervention efforts and manage emergency situations with professionalism and sensitivity.
- Support families through child & family counseling, special education advocacy, or Alzheimer's care planning as needed.
- Collect data for clinical research initiatives and contribute to ongoing quality improvement projects.
- Assist with case management activities such as discharge planning, resource linkage, and follow-up services.
- Maintain accurate medical records using appropriate medical terminology and coding systems (e.g., CPT coding, ICD-10/ICD-9/ICD coding).
- Engage in ongoing professional development related to psychopharmacology, crisis management, and mental health counseling best practices.

- Up to date resume that includes 2+ year(s) of Social Work experience. Also, must document travel experience. First time travelers will be considered State Licensed social work license (LCSW or equivalent - LICSW or LMHC)
