  • JOB SUMMARY
    We are seeking a Registered Nurse (RN) for home healthcare services to conduct patient assessments and deliver quality care through both field visits and telehealth. This role focuses on patient safety, accurate evaluations, and flexibility in managing daily schedules while supporting a long-term healthcare program.

    KEY RESPONSIBILITIES
    • Conduct in-home patient assessments and telehealth evaluations  
    • Perform UAS assessments where applicable  
    • Deliver high-quality patient care with a focus on safety and compliance  
    • Complete approximately 3 patient visits per day, each lasting 1.5–2 hours  
    • Maintain accurate documentation of patient assessments and care plans  
    • Travel within assigned local regions for patient visits  
    • Collaborate with healthcare teams to ensure continuity of care  
    • Manage schedule flexibly based on assigned assessments  
    REQUIRED QUALIFICATIONS
    • Active Registered Nurse (RN) license in New York State  
    • Minimum 2 years of recent home healthcare experience  
    • Ability to perform field visits and telehealth assessments  
    • Strong clinical assessment and documentation skills  
    • Ability to travel locally within assigned regions  
    • Strong communication and patient care skills  
    • Ability to work independently with flexibility  
    PREFERRED QUALIFICATIONS
    Experience with UAS assessments
