Job Summary
The RN Case Manager is responsible for facilitating effective discharge planning and ensuring continuity of care for patients. This role requires close collaboration with healthcare teams and insurance providers to perform utilization reviews and manage patient cases efficiently.
Key Responsibilities
- Coordinate discharge plans and continuity of care
- Perform utilization reviews and maintain case documentation
- Collaborate with insurance providers and interdisciplinary hospital teams
Required Qualifications
- Active RN license in California
- Prior experience in hospital case management
- Familiarity with EMR systems and discharge planning
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
Preferred Qualifications
- CCM or ACM certification
Certifications
- Active RN license in State.
- CCM or ACM certification (preferred)
