JOB SUMMARY
Provides direct patient care and support under the supervision of a Registered Nurse.

Key Responsibilities
- Administer medications and treatments as prescribed.
- Monitor patient vital signs and condition, reporting any changes to the Registered Nurse.
- Assist with patient assessments and care planning.
- Perform wound care and dressing changes.
- Educate patients and families on health conditions and treatments.
- Document patient care in accordance with facility policies.
- Assist with admissions, transfers, and discharges.

Schedule / Shift
Weekends and holidays as scheduled.

Required Qualifications
- Valid state Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.

Experience
- Previous nursing experience preferred.

Skills
- Proficient in patient assessment and care planning.
- Strong understanding of medication administration and therapeutic procedures.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.

EMR / Systems
- Familiarity with electronic health record (EHR) systems is beneficial.

Patient Population
- Adults and/or pediatrics, depending on assignment.

Unit / Specialty
- Medical-Surgical
- Other units as assigned.

Floating / On-Call
- May be required to float to other units based on facility needs.
