Job Summary
The Registered Nurse – Home Health provides skilled nursing care to patients in their homes, ensuring safe, effective, and compassionate treatment. This role involves conducting patient assessments, administering medications, coordinating care plans, and educating patients and families on managing health conditions. The RN works independently in the field while collaborating with physicians, therapists, and other healthcare professionals to promote optimal patient outcomes.
Key Responsibilities
- Perform comprehensive patient assessments in the home setting
- Develop, implement, and evaluate individualized care plans
- Administer medications, wound care, IV therapies, and other skilled nursing interventions
- Educate patients and families on disease management, medication use, and lifestyle modifications
- Monitor patient progress and communicate changes to physicians and interdisciplinary team members
- Document patient care accurately in electronic medical records (EMR)
- Ensure compliance with infection control, safety, and regulatory standards
- Provide compassionate support and advocacy for patients and families
Required Qualifications
- Current RN license in the state of practice
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) with commitment to obtain BSN
- Minimum of 1–2 years of clinical nursing experience
- Strong assessment, critical thinking, and communication skills
- Ability to work independently and manage multiple patient visits daily
- Reliable transportation and willingness to travel to patient homes
Preferred Qualifications (if any)
- Prior experience in home health, hospice, or community nursing
- Familiarity with electronic medical records (EMR) systems
- Knowledge of Medicare/Medicaid regulations and home health documentation requirements
Certifications (if any)
- Basic Life Support (BLS) – required
- Advanced Cardiovascular Life Support (ACLS) – preferred
- Specialty certification in Home Health Nursing (CHPN or similar) – preferred
