  • Job Summary:
    The Registered Nurse - Diabetes Educator provides compassionate, culturally sensitive, and holistic nursing care to patients in a clinical setting in alignment with the Mercyhealth nursing mission, values, and strategic plan. In collaboration with a multidisciplinary diabetes management team, the RN Diabetes Educator delivers comprehensive diabetes education to patients and families living with diabetes mellitus. The role may include variable shifts, weekends, holidays, on-call hours, and floating to various units based on organizational needs and clinical competency.

    Key Responsibilities:
    • Utilize the nursing process and collaborate with interdisciplinary team members to deliver patient-centered care
    • Provide diabetes education to patients and families in coordination with the diabetes management team
    • Administer prescribed medications and treatments in accordance with nursing standards, policies, procedures, and state practice acts
    • Develop, initiate, and execute individualized patient plans of care as prescribed by physicians and/or clinic policy
    • Provide patient and family education to support disease management and self-care
    • Document patient care according to standards of care and regulatory requirements
    • Participate in department and unit performance improvement activities, including shared governance
    • Provide clinical expertise and guidance to other units and departments to support longitudinal patient care plans
    • Utilize critical thinking to ensure patient safety and positive patient experience, escalating concerns through appropriate channels
    • Participate in the professional nursing continuum process
    • Act as a resource for staff regarding patient education programs, materials, and related issues
    • Advocate for patients by addressing clinical, psychosocial, and financial concerns
    • Float to various units as needed based on identified needs and clinical competency
    • Perform other duties as assigned

    Education:
    Graduate of an accredited nursing program
    BSN preferred

    Certifications:
    Active RN license in the state of Illinois
    Active RN license in the state of Wisconsin
    Approved CPR certification required within 90 days of hire
    CDCES (Certified Diabetes Care and Education Specialist) certification required
