Job Summary
• Provide professional nursing care and care coordination for patients across various healthcare settings
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes and quality care
• Serve as an advocate and resource for patients, families, and healthcare providers
Key Responsibilities
• Conduct comprehensive assessments of patients’ physical, psychological, and social needs
• Develop, implement, and evaluate individualized care plans
• Coordinate transitions of care across inpatient, outpatient, and home settings
• Educate patients and families on treatment plans, medications, and self-care strategies
• Collaborate with providers, specialists, and community resources to support patient care
• Monitor patient progress and adjust care plans as needed
• Maintain accurate documentation and comply with regulatory standards
• Participate in quality improvement initiatives and data analysis
Required Qualifications
• Registered Nurse (RN) license in good standing
• Associate’s or Bachelor’s degree in Nursing
• Minimum of 2 years of clinical nursing experience
• Experience with care coordination or case management
• Strong communication, organizational, and critical thinking skills
• Proficient in electronic health records (EHR) systems
Preferred Qualifications
• Bachelor’s degree in Nursing (BSN)
• Experience in chronic disease management or population health
• Previous work in outpatient or community health settings
Certifications
• Basic Life Support (BLS) Certification
• Certified Case Manager (CCM) preferred
