  • Job Summary
    We are seeking a compassionate and skilled Registered Nurse with Home Health experience to join our care team. This role involves providing high-quality, patient-centered care in a home setting, supporting individuals with chronic or terminal illnesses. The ideal candidate is experienced in hospice care and comfortable working independently in a remote environment.

    Key Responsibilities
    1. Deliver professional nursing care to homebound patients in accordance with individualized care plans.
    2. Monitor and assess patient conditions and report changes to the interdisciplinary care team.
    3. Administer medications and treatments as prescribed.
    4. Provide emotional support and education to patients and families.
    5. Coordinate care with other healthcare providers and document all activities accurately in the EPIC system (Remote Client).
    6. Ensure compliance with all clinical and regulatory standards.

    Required Qualifications
    1. Active and valid Certified Home Health Aide (HHA) certification
    2. Basic Life Support (BLS) certification
    3. Prior experience in hospice or palliative care settings
    4. Familiarity with EPIC (Remote Client) or similar EMR systems
    5. Excellent communication and time management skills

    Preferred Qualifications
    1. Additional training or coursework in palliative or end-of-life care
    2. Strong interpersonal and teaching abilities

    Certifications
    1. Certified Home Health Aide (HHA)
    2. Basic Life Support (BLS)
