JOB SUMMARY

The Patient Care Coordinator is responsible for overseeing the management and coordination of care for the acute inpatient population. This role involves collaborating with physicians, patients, families, nursing staff, utilization review teams, and other healthcare professionals to ensure continuous patient care progression, promoting clinical and cost-effective outcomes. The Patient Care Coordinator also facilitates and coordinates with community providers and ambulatory case managers to assist with appropriate levels of care transitions, facilitate safe discharges, and prevent readmissions.

Key Responsibilities

• Discharge planning, ensuring patients have appropriate discharge plans.
• Collaborating with physicians and the ancillary team.
• Working with patients and families to create discharge plans.
• Collaborating with rounding physicians.
• Assuring continuum of patient care progression for clinical and cost-effective outcomes.
• Facilitating and coordinating with community providers and ambulatory case managers.
• Assisting with the appropriate level and transition of care for a safe discharge.
• Preventing readmissions.
