  • Job Summary
    The Nurse Navigator Registered Nurse (RN) is responsible for guiding patients through the healthcare continuum by coordinating care, facilitating access to resources, and providing education and support. The RN serves as a patient advocate, helping individuals and families navigate complex healthcare systems while promoting quality outcomes, patient satisfaction, and continuity of care.

    Key Responsibilities
    1. Coordinate and manage patient care across multiple departments, providers, and healthcare settings.
    2. Serve as a primary point of contact for patients and families throughout the care journey.
    3. Assess patient needs and identify barriers to care, treatment, and follow-up services.
    4. Educate patients and caregivers regarding diagnoses, treatment plans, procedures, and available resources.
    5. Facilitate appointments, referrals, diagnostic testing, and care transitions.
    6. Collaborate with physicians, case managers, social workers, and interdisciplinary healthcare teams.
    7. Monitor patient progress and ensure timely follow-up and adherence to treatment plans.
    8. Provide emotional support, advocacy, and guidance to patients and families.
    9. Maintain accurate and timely documentation in the electronic medical record (EMR).
    10. Participate in quality improvement initiatives, patient satisfaction programs, and care coordination activities.
    Required Qualifications
    1. Active Registered Nurse (RN) license in the applicable state.
    2. Graduate of an accredited nursing program.
    3. Minimum of one (1) year of clinical nursing experience in Acute Care, Case Management, Care Coordination, Oncology, or a related specialty preferred.
    4. Strong knowledge of patient care coordination, healthcare systems, and clinical practices.
    5. Excellent communication, organizational, and interpersonal skills.
    6. Ability to manage multiple priorities and work independently.
    7. Proficiency with electronic medical records (EMR/EHR) systems.
    Preferred Qualifications
    1. Bachelor of Science in Nursing (BSN).
    2. Previous experience as a Nurse Navigator, Case Manager, Care Coordinator, or Patient Advocate.
    3. Experience in specialty areas such as Oncology, Cardiology, Women's Health, or Population Health.
    4. Knowledge of community resources, insurance processes, and care transition management.
    Certifications
    1. Basic Life Support (BLS) Certification.
    2. Certified Case Manager (CCM) Certification, preferred.
    3. Nurse Navigator Certification (NNCP), preferred.
