  • Job Summary  
    We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our Hospice Home Health team. The LVN will provide high-quality nursing care to patients in their homes, supporting comfort, dignity, and quality of life during end-of-life care. This role requires strong clinical skills, empathy, and the ability to work collaboratively with an interdisciplinary care team.

    Key Responsibilities
    1. Deliver skilled nursing care under the supervision of a Registered Nurse (RN).
    2. Monitor patient conditions and promptly report changes to the care team.
    3. Administer prescribed medications and treatments accurately.
    4. Provide emotional support to patients and families, fostering a compassionate environment.
    5. Assist with patient and family education regarding care plans and symptom management.
    6. Document patient care activities in compliance with regulatory and organizational standards.
    7. Collaborate with physicians, social workers, and other team members to ensure comprehensive patient care.

    Required Qualifications
    1. Current Licensed Vocational Nurse (LVN) license in the state of practice.
    2. Minimum of 2 year of nursing experience, preferably in hospice, home health, or palliative care.
    3. Strong communication and interpersonal skills.
    4. Ability to work independently and as part of a team.
    5. Reliable transportation and willingness to travel to patient homes.

    Preferred Qualifications (if any)
    1. Prior hospice or palliative care experience.
    2. Familiarity with electronic medical records (EMR) systems.
    3. Training in pain and symptom management.

    Certifications (if any)
    1. Basic Life Support (BLS) certification required.
    2. Hospice and Palliative Care Nursing certification (CHPN) preferred.
