Job Summary
We are seeking a compassionate and dedicated Social Worker to provide support and resources to individuals and families facing various social, emotional, and economic challenges. The Social Worker will assess client needs, develop care plans, and coordinate with community resources to promote well-being and improve quality of life.
Key Responsibilities
- Conduct thorough assessments of clients’ social, emotional, and financial needs.
- Develop and implement individualized care plans to address client goals and challenges.
- Provide counseling and emotional support to clients and families.
- Connect clients with community resources, such as housing, healthcare, employment, and financial assistance programs.
- Advocate for clients to ensure access to services and benefits.
- Maintain detailed and accurate case records in compliance with agency policies and regulations.
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal professionals.
- Participate in community outreach and education programs.
- Monitor and evaluate client progress and adjust care plans as necessary.
- Ensure confidentiality and ethical standards in all client interactions.
Required Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
- Current state licensure or certification as a Social Worker.
- Strong interpersonal and communication skills.
- Ability to handle sensitive and confidential information with professionalism.
- Knowledge of community resources and social services systems.
- Experience in case management and client advocacy.
- Ability to work independently and as part of a team.
Preferred Qualifications
- Experience working with diverse populations including children, elderly, or individuals with disabilities.
- Crisis intervention and conflict resolution skills.
- Bilingual or multilingual abilities.
Certifications
- Licensed Clinical Social Worker (LCSW) or equivalent preferred.
- CPR and First Aid certification (if applicable)
