  • Job Summary
    We are seeking a dedicated Social Worker II to provide compassionate social services to patients and their families. This role involves conducting psychosocial assessments, discharge planning, crisis intervention, and serving as a liaison between patients, families, hospital staff, and community resources.

    Key Responsibilities
    1. Provide social services to patients and families for assigned medical services.
    2. Conduct discharge planning evaluations and gather necessary information.
    3. Perform psychosocial assessments and communicate findings to hospital staff.
    4. Counsel patients and families regarding diseases, treatments, financial or social situations, and emotional stress.
    5. Identify high-risk cases and provide crisis counseling related to child abuse, neglect, or physical abuse.
    6. Deliver crisis intervention services and document sessions appropriately.
    7. Act as a liaison between patients/families, hospital staff, and external agencies.
    8. Evaluate the effectiveness of services provided and adjust care plans as needed.
    9. Assist with discharge planning, referrals, and post-hospital placement or transfer.
    10. Support patients and families in adjusting to illness, treatment, or changes in functional status.
    11. Utilize specialized interviewing and communication skills, particularly with children.
    12. Advocate for patients in areas such as informed decision-making, end-of-life care, and ethical dilemmas.
    13. Prepare statistics related to referrals, extended stays, and other elements as required.
    14. Supervise students or newly hired employees when assigned.
    15. Must have at least 2 years of recent experience in the specialty within the past 2 years.

    Required Qualifications
    1. Master’s Degree in Social Work from an accredited institution.
    Preferred Qualifications
    1. Licensed Master Social Worker (LMSW) strongly preferred.
    2. Prior hospital or healthcare experience preferred.

    Certifications
    1. LMSW or equivalent state licensure (preferred).
