Job Summary
We are seeking a dedicated Social Worker II to provide compassionate social services to patients and their families. This role involves conducting psychosocial assessments, discharge planning, crisis intervention, and serving as a liaison between patients, families, hospital staff, and community resources.
Key Responsibilities
- Provide social services to patients and families for assigned medical services.
- Conduct discharge planning evaluations and gather necessary information.
- Perform psychosocial assessments and communicate findings to hospital staff.
- Counsel patients and families regarding diseases, treatments, financial or social situations, and emotional stress.
- Identify high-risk cases and provide crisis counseling related to child abuse, neglect, or physical abuse.
- Deliver crisis intervention services and document sessions appropriately.
- Act as a liaison between patients/families, hospital staff, and external agencies.
- Evaluate the effectiveness of services provided and adjust care plans as needed.
- Assist with discharge planning, referrals, and post-hospital placement or transfer.
- Support patients and families in adjusting to illness, treatment, or changes in functional status.
- Utilize specialized interviewing and communication skills, particularly with children.
- Advocate for patients in areas such as informed decision-making, end-of-life care, and ethical dilemmas.
- Prepare statistics related to referrals, extended stays, and other elements as required.
- Supervise students or newly hired employees when assigned.
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
Required Qualifications
- Master’s Degree in Social Work from an accredited institution.
Preferred Qualifications
- Licensed Master Social Worker (LMSW) strongly preferred.
- Prior hospital or healthcare experience preferred.
Certifications
- LMSW or equivalent state licensure (preferred).
