Job Summary
The Certified Social Worker provides professional support to individuals, families, and communities by assessing needs, developing care plans, and connecting clients with appropriate resources. This role focuses on advocacy, counseling, and case management to promote well-being and improve quality of life.
Key Responsibilities
- Conduct assessments to identify client needs and develop individualized care plans.
- Provide counseling and emotional support to clients and families.
- Coordinate with healthcare providers, community organizations, and government agencies to ensure access to resources.
- Advocate for clients’ rights and assist them in navigating social service systems.
- Maintain accurate documentation and case records in compliance with regulatory standards.
- Monitor client progress and adjust care plans as needed.
- Participate in multidisciplinary team meetings to support holistic care.
- Educate clients and families on available programs, services, and coping strategies.
Required Qualifications
- Bachelor’s degree in Social Work (BSW) or related field.
- Certified Social Worker (CSW) credential.
- Strong communication and interpersonal skills.
- Ability to manage multiple cases and prioritize effectively.
- Knowledge of social service systems, community resources, and client advocacy practices.
- High level of empathy, professionalism, and cultural sensitivity.
Preferred Qualifications (if any)
- Master’s degree in Social Work (MSW).
- Prior experience in healthcare, mental health, or community-based social work.
