Job Description:

  • JOB SUMMARY
    Provide social work services to patients and families to address psychosocial issues impacting health and well-being. Support patient care through assessment, counseling, resource identification, and advocacy.

    Key Responsibilities
    - Conduct psychosocial assessments to identify patient and family needs related to illness, disability, and life changes.
    - Provide counseling and support to patients and families facing emotional, social, and financial challenges.
    - Develop and implement social work intervention plans in collaboration with the interdisciplinary care team.
    - Connect patients and families with community resources, support groups, and financial assistance programs.
    - Advocate for patient rights and facilitate access to necessary services.
    - Document all assessments, interventions, and progress notes in patient medical records.
    - Participate in interdisciplinary team meetings to discuss patient care plans.
    - Respond to patient and family needs in a timely and compassionate manner.

    Schedule / Shift
    Friday - Monday, 8:00 AM - 5:30 PM

    Required Qualifications
    - Master's degree in Social Work (MSW) from an accredited program.
    - State licensure as a Social Worker.

    Experience
    - Previous experience in a healthcare or hospital setting is preferred.

    Certifications
    - LCSW (Licensed Clinical Social Worker) or equivalent clinical license preferred.

    Skills
    - Strong assessment and counseling skills.
    - Excellent communication and interpersonal skills.
    - Proficient in crisis intervention and case management.
    - Knowledge of community resources and social service systems.
    - Ability to work independently and as part of a team.

    EMR / Systems
    - Experience with electronic health record systems.

    Patient Population
    - Broad adult patient population.

    Unit / Specialty
    Social Services

    Floating / On-Call
    - May be required to float to other related departments based on facility needs.
