  • Job Summary
    We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide in-home healthcare services. The ideal candidate will have strong clinical skills, attention to detail, and the ability to deliver high-quality care in a home health setting. This role requires experience with Epic Homecare, strong communication abilities, and a proactive approach to patient care.

    Key Responsibilities
    • Evaluate and monitor patients’ progress in the home setting
    • Identify and respond to sudden or subtle changes in a patient’s medical condition
    • Communicate updates clearly to physicians, patients, and family members
    • Begin treatment and monitor medication dosages
    • Respond to medical emergencies and notify appropriate healthcare providers
    • Complete all necessary documentation prior to patient transfers
    • Maintain up-to-date and accurate patient records
    • Develop and implement effective, individualized care plans
    Required Qualifications
    • Education: Bachelor of Science in Nursing (BSN)
    • Experience: Minimum 2 years of current LPN experience, specifically in Med Surg unit 
    • Technical Skills: Experience with Epic Homecare system
    • Licensure: Active LPN License
    Certifications:
    • Basic Life Support (BLS) – Required
    • COVID-19 vaccination – Required

    Preferred Qualifications
    • Strong interpersonal and communication skills
    • Ability to work independently in a home-based care environment
    • Experience handling medical emergencies in a non-hospital setting
    • Organizational skills and attention to documentation accuracy

    Certifications
    • LPN License (if required for position)
    • BLS Certification – Required
